The patient was the driver of an all-terrain vehicle that rolled off the road and ejected him into a ditch. When a paramedic team arrived at the scene, the patient had a blood pressure of 90/50 mmHg, a heart rate of 120 bpm, a respiratory rate of 24 breaths per minute, and an oxygen saturation of 95% without supplemental oxygen. His Glasgow Coma Scale score was 7 (opened eyes to pain-2, moaned-2, abnormal flexion-3). The patient was given oxygen via nasal prongs and a non-rebreathing face mask, intravenous access was obtained, and an infusion of crystalloid was begun.
